How I handle the information you give me — and what I will never do with it.
What I collect
When you use a form on this site, I collect your first name, last name, email address, phone number, and ZIP code. I collect nothing else. There is no tracking pixel, no advertising cookie, and no third-party analytics script that identifies you.
Why I collect it
So that Terry Steaple, a licensed insurance agent, can contact you about health insurance options. That is the only reason.
What I will never do
- I do not sell your information.
- I do not share it with lead buyers, aggregators, or marketing networks.
- I do not use it to advertise to you anywhere else.
Where it goes
Your information is stored in GoHighLevel, the customer relationship platform I use to manage inquiries, and is accessible only to Terry and to people working directly on his behalf to respond to you.
How to reach us, or ask us to delete it
Call the number in the footer of this page, or email terry@mrterryhealthinsurance.com. Ask me to delete your information and I will.
Health information
This site never collects a Medicare or insurance member ID number, a health condition, or plan details — not through this form, not anywhere on this site.
Text messages (SMS)
If you opt in to receive text messages from Mr Terry Health Insurance, the following applies:
- Message frequency varies. You may receive appointment reminders, follow-ups related to your inquiry, or service updates.
- Message and data rates may apply, depending on your mobile carrier and plan.
- Text STOP to any message to opt out at any time. You will receive a single confirmation and no further messages.
- Text HELP for assistance, or call (850) 321-2930.
- Your mobile opt-in data — including your phone number and your consent to receive texts — will never be shared with or sold to third parties or affiliates for their own marketing purposes.
By opting in, you confirm that you are the subscriber or customary user of the mobile number provided. Consent to receive text messages is not a condition of purchasing any product or service.
